To cancel this transaction, mail, fax or deliver a signed and dated copy of this cancellation notice or any other written notice, or send a telegram to Gold Star Referral Clubs, 4938 E. 73rd Street, Tulsa, Ok 74136, NO LATER THAN MIDNIGHT ON the third business day after the date of this transaction. You must state: I HEREBY CANCEL THIS TRANSACTION. And the cancellation must have Buyer's (your) Signature.
NO REFUNDS ARE GIVEN AFTER THE THREE BUSINESS DAYS HAVE PASSED from the date of purchasing the Director Kit and becoming a Gold Star Director. THERE ARE NO EXCEPTIONS.
Procurement of a consumer and/or investigative report
By submitting this application, you understand that Gold Star Referral Clubs may, through an independent contractor, Steiner Consulting 9524 E 81st Suite B #1562, Tulsa, OK 74133 procure an investigative or consumer report on you. You also understand that at Gold Star's discretion, during the term of your association with Gold Star, subsequent consumer or investigative reports may be procured. These above-mentioned reports may include a social security number verification.
You are entitled to a complete and accurate disclosure of the nature and scope of any investigative report of which you are the subject upon your written request to Steiner Consulting, 9524 E 81st Suite B #1562, Tulsa, OK 74133. Your independent contractor position with Gold Star may be contingent upon the successful completion of the above-referenced reports.
BUILDING A GOLD STAR BUSINESS
You can build a Gold Star business by starting and maintaining Gold Star Referral Clubs and also by sponsoring other Directors to do the same. Gold Star publishes an authorized Compensation Plan, which outlines the benefits and requirements of a Gold Star business. INDEPENDENT CONTRACTOR STATUS
Gold Star Directors are INDEPENDENT CONTRACTORS. Independent contractors are not employees of Gold Star, and may not represent themselves as employed by Gold Star. The Director will not be treated as an employee for federal or state tax purposes. Nor will the Director be treated as an employee for purposes of the Federal Insurance Contributions Act or any other laws covering employees.
Gold Star Directors may not make claims about Gold Star products or Compensation Plans that are contrary to literature and published material by Gold Star Referral Clubs. The Policies and Procedures are amended from time to time and is incorporated in this agreement. Please review it carefully with your Sponsor or Gold Star upline. It sets forth the foundation of how we do business. If a Gold Star Director does not adhere to the Policies and Procedures, they are subject to remedies for breach of contract, including termination of their Gold Star Directorship and Gold Star Club membership and all the benefits thereof.
